A source is blamed because the candidates are weak. A vendor is blamed because the ATS count is wrong. The ATS is blamed because the campaign platform shows more conversions. Each explanation may contain part of the truth.
Before deciding who failed, reconstruct the handoff.

A problem can enter at one boundary and appear at another. Incorrect source parameters may look like ATS reporting failure. A recruiter capacity issue may look like poor candidate interest. A hiring-manager delay may make a campaign appear unable to convert.
Use the Groundwork handoff review
- Name what was expected to enter the handoff.
- Name what the receiving system or person actually received.
- Identify the owner on both sides.
- Document the timing and acceptance criteria.
- Review exceptions and manual workarounds.
- Confirm what evidence exists when the handoff is disputed.
A handoff is not complete because data moved. It is complete when the receiving party can use what arrived for the intended decision.
Look for silent transformations
Integrations often rename fields, overwrite values, merge events or drop information. Human handoffs do the same thing through interpretation. A campaign objective becomes a volume goal. A minimum requirement becomes a preference. A vendor limitation becomes an internal workaround that no one prices or documents.
Ask the receiving team
Dashboards show whether an event occurred. The receiving team can tell you whether it was usable. Ask recruiters what arrives incomplete, hiring managers what evidence they do not trust and operations teams where they repeatedly intervene by hand.
Assign the correction where authority exists
Do not assign a recruiter responsibility for an integration they cannot change or a media partner responsibility for an ATS automation they cannot see. The owner of the correction must have both visibility and authority. Otherwise the organization is only relocating accountability.
What to document after the review

- The temporary control
- The permanent owner
- The deadline
- The validation test
Reconstructing the handoff does not excuse poor performance. It makes performance addressable. Once the chain is visible, the organization can fix the actual failure instead of negotiating between competing stories.
